Respite Connections
Purpose:
To provide a service to families to access relief supports in
their own home and/or community.
SNAPS will:
- Develop a list of potential respite care providers
- Develop a list of families requiring respite care
- Assist in the matching of families and care providers
- Provide support and resources to families and care providers
Families will:
- Be able to access a list of care providers and interview for the most suitable match for their family member
- Be informed as to the interview process and make an informed decision with regards to respite care
- Feel empowered to employ their own care provider, feel confident to train the care provider and communicate their wishes to the care provider
- Be confident in completing paperwork to access funding (ie. FSCD) and pay the care provider
- Decide whether the family wishes respite to occur in their own home, the community or the care provider’s home
- Remain focused on respite care as an enjoyable, fun activity for their family member and it is to provide care so that the family can have a break
Respite Care Providers will:
- Provide a police record check and child welfare check
- Provide a resume and three references
- Initially be screened by SNAPS then interviewed by the family to ensure a good match is found
- Provide information on availability
- Complete a training in service on respite care
